Sperm Concentration
The number of sperm per millilitre. Normal is defined as at least 16 million. Thresholds below this are classified as Oligospermia or Azoospermia.
Sperm Motility
The ability of sperm to swim forward. Effective natural fertilisation requires at least 30% progressive motility.
Sperm Morphology
The structural shape of the sperm. Using strict Kruger criteria, the normal form threshold is at least 4%.
Advanced: Sperm DNA Fragmentation
Measures the integrity of genetic material. High DNA fragmentation can impair embryo development even when standard parameters appear normal. Crucial for unexplained infertility or repeated IVF failure.
Common Causes
Varicocele
Dilation of veins in the scrotum that raises testicular temperature. Present in 40% of infertile men, it is the most common surgically correctable cause.
Hormonal Disorders
Abnormalities in Testosterone, FSH, or Thyroid hormones. Hypogonadism is a frequently treatable condition through targeted hormone therapy.
Azoospermia
Distinguishing between Obstructive (blockage) and Non-Obstructive (production) is vital. Surgical retrieval like TESA or Micro-TESE offers high success rates.
Genetic Factors
Chromosomal abnormalities like Klinefelter Syndrome. Genetic testing is essential for treatment planning and safeguarding offspring.
Environment & Habits
Smoking, obesity, scrotal heat exposure (laptops), and anabolic steroids are severe suppressors of sperm production. Many of these factors are reversible within 3-6 months.

Treatment depends entirely on the identified cause. We begin with lifestyle modifications and medical management where possible. Surgical correction of Varicocele can restore fertility in a meaningful proportion of men.
For severe cases, IVF with ICSI is the definitive treatment. It requires only a single healthy sperm per egg. For men with zero sperm count, surgical sperm retrieval combined with ICSI offers the possibility of biological fatherhood.
